Nathan's journey into physiotherapy began amidst his active pursuits of basketball and rugby, where he excelled to a high level. Armed with a degree in Sports Science, his passion led him to pursue further studies at the Masters level in Strength and Conditioning, propelling him into the realm of professional sports.

During his tenure working closely with Physiotherapists and Doctors, Nathan recognised the limitations of following strict protocols, prompting him to embark on a Masters in Physiotherapy at Robert Gordon University. This enabled him to embark on a remarkable journey, initially working with Munster Rugby and subsequently in the United States at prestigious hospitals like Washington Hospital Centre and Walter Reed Military Hospital, specialising in orthopaedics and amputee rehabilitation. He also served as the director of outpatients at a Private clinic in Washington DC.

Nathan's expertise extended to the sports arena, where he worked with the Irish Rugby league team and consulted in the NFL with the Philadelphia Eagles.

  • INJECTION THERAPY
    Injection therapy utilising corticosteroid or ostenil injections to help with a variety of joint and neuromuscular conditions . It is not a stand alone treatment but together with other modalities can provide relief for months or even years. It is commonly used in the treatment of arthritis or bursitis. Corticosteroid can help by reducing inflammation and thus providing a period of pain relief in order to actively engage in rehabilitation. Ostenil is the injection of synovial fluid into a joint. All our joints are lubricated with synovial fluid but over time and through the natural ageing process or via trauma this can leak out of a joint. By lubricating the joint you are able to reduce friction of the joint thus reducing inflammation.   • NEUROMUSCULAR PHYSIOTHERAPY
    Specialising in the treatment of neuromuscular conditions. This is done using a variety of treatment tools available to us. Nathan offers advice and education to empower his patients in the management of their condition. Exercise therapy, the foundation of our treatment approach to restore normal function, this can be either home, pool or gym based. Manual therapy including joint manipulations and mobilisations to get things moving, when they are stuck. Nathan is also specialised in amputee rehabilitation and can provide support and rehabilitation step by step to reach goals and return to every day activities and work. Nathan is also able to prescribe as sometimes pain relief is needed to settle things down to engage in the physiotherapy.     Our body composition scans are created by multiple infrared screenings within just 35 seconds, forming a virtual 3D model of your body.     If you have spent years yo-yo dieting, and are controlled by your food, calories, low-fat and reduced-sugar foods and the number on the bathroom scales, working with our nutritionist can help you gain back your control. You may have disordered eating or an eating disorder. You may recently have had a Health Screening where BMI, High Blood Pressure, Cholesterol or diabetes risk may have been raised as concerns by the GP. This package is designed to set you off on the right path to help you address these issues yourself naturally as opposed to medication. Our nutritionist can help you change your mindset and understanding of the food you provide to the amazing machine called your human body. During the weight management consultation sessions, you will be given tools and knowledge to empower you to take back your control and live without focusing on a single calorie. You may have blood sugar imbalances and insulin resistance or be pre-diabetic with metabolic syndrome and have been told that you need to lose weight. You may have cardiovascular issues and have been told that weight management will help to support your health and well-being. But you just do not know where to start and are not inspired by going to a slimming club. All these problems can be helped with bespoke nutritional support.     If you are suffering perimenopause or menopausal symptoms and would like holistic support to help you through this natural life transition, then consider working with our in-house nutritionist and wellbeing expert.     If you suffer headaches, migraines, brain fog, stress, joint pain, skin conditions or inflammation. Our resident nutritionist can work with you to help you achieve optimal health. If you are plagued with IBS or IBD and your symptoms such as bloating, gas, indigestion, heartburn and abdominal pain control you, nutritional support using a combination of functional medicine and Ayurveda lifestyle principles may be the answer to your problems. There is no set protocol during your consultations as each person is unique.   • Counselling - £70
    Each session is designed to last for 60 minutes. You can determine the number of sessions and the frequency. You are in control of the number of appointments you would like to have. As a general rule, every 6th session will contain a 20 minute “review” to appraise the work, discuss progress and review your goals. Please note our counselling services are only suitable for adults and older children aged 16 and over.   • Do you do home visits?
    Yes – depending on the membership plan you are on and whether you are in our home visit catchment area. Fees may apply if you have used up your annual plan entitlement or are outside of our catchment area.
  • Do I need to book an appointment or can I just walk in?
    Appointments are strictly by prior appointment. If you are a member, you can book appointments online through our patient portal on our website.
  • Will my NHS GP know that I am registered with you?
    No – not unless you consent for us to keep them updated. Any routine investigations and test results are private to you and will not be shared with your GP. Only with your permission will we share your treatment with us with your registered GP. For your own health and safety, we will ask for your consent to share important medical information with your GP.
  • Can you refer me to see a private specialist?
    Yes – we have access to a local network of private specialists and we can refer you to see these. If you have private health provision, these are normally covered by your insurance. If you do not, we can refer you to see one on a self-pay basis. Please note that payment arrangements are direct between you and the specialist, private clinic or private hospital – these are not included in our fees.
  • Do I need to join your practice to be seen by a private GP?
    Ideally you will be signed up as a member on an annual monthly subscription plan. However, you can register with us as a Pay As You Go patient.
  • What if I have a medical emergency?
    You will need to call 999 or 111 as we are not equipped to respond to these. We only cover routine check-ups.
  • Can you give me an NHS prescription?
    No, we can only issue you with a private prescription that your pharmacy will process in the same way. Some medications are cheaper on a private prescription than with the NHS and others are more expensive. This can vary from pharmacy to pharmacy. We will aim to have a pharmacy that can deliver medication to you. You can still get your NHS prescriptions that your NHS GP has prescribed you.
  • Is a private GP covered by my medical insurance?
    No, this service is not covered by most private medical insurance plans – you will need to pay for this yourself.
  • Can I see a GP and not be on an annual membership plan?
    Yes, you can pay as you go for the service that you use. Please note that these services come at an increased price and we prioritise our appointments for membership plan patients. You can join as a member on an annual plan at any time, which may be more cost effective in the longer term depending on your healthcare needs.
  • Is there an initial contract term for your membership plans?
    Yes - 6 months. Thereafter you can cancel.
  • How do I pay?
    You can pay our fees securely by credit or debit card when you book online or over the phone. If you are on a subscription plan, the monthly fee is taken from the card that was used to make the initial booking.
  • What are your opening hours?
    Monday: 08:30 - 16:30 Tuesday: 08:30 - 16:30 Wednesday: 08:30 - 16:30 Thursday: 09:30 - 18:30 Friday: 08.30 - 15:30 / 18:30 (alternate weeks) Saturday: 09:30 – 12:30 Sunday: Closed Out of these hours, we will have a call answering service that will forward messages to our team and we will call you back at the start of the next working day. In the interim, you will be able to book an appointment to see a GP online subject to availability. As we grow our patient list, we will be able to extend our clinic times and expand our service offering.
  • What happens if I think I or a family member has coronavirus?
    Please follow the government guidelines on self-isolation and quarantine. For the safety of our doctors, staff and other patients, all suspected coronavirus cases are dealt with by the NHS. Please call 111 immediately if you are having difficulty breathing or it is a medical emergency.
  • Do I need to pay to join?
    There is no joining fee. Our annual membership plans include a range of services that are included in your monthly subscription fee and are therefore free to use. If you choose our pay as you go option, you pay for the service at the time of booking.
  • What are the different membership plans?
    We have a Platinum, Premium and Essential plan. Additionally, we offer a Corporate plan for businesses. They have all have single, couple, family and children membership options so that you can create the plan you need, depending on your family circumstances. There is no joining fee and the annual plans have a number of services included, depending on which plan you choose.
  • Can I still be registered with my NHS GP and use a private GP?
    Yes, you can. You do not forfeit any of your NHS entitlements by using our service. We recommend that you stay registered with your NHS GP practice to ensure assistance in the event of a medical emergency.
  • What about my medical history?
    At the present time, we will not have access to your NHS medical records, but part of your registration process is for the GP to capture this information from you.
  • Can I have a chaperone?
    Yes – we respect your rights, privacy, religious and cultural beliefs, confidentiality and dignity. You can request a chaperone when you arrive for your appointment.
  • If I join, should I leave my NHS GP?
    We recommend that patients stay registered with their NHS GP.
  • Can you refer me to an NHS specialist or clinic?
    No, we do not have access to do this. We can only refer you to NHS A&E.
  • How long will I need to wait to see a GP?
    There are no long waiting times and we can usually provide same day consultations. We aim to offer appointments within 24 hours to our membership patients.
  • Do you write sick notes?
    Yes, if this is required following your consultation with the GP. If you are on a membership subscription, this is included within your plan. They are also available on a pay as you go basis for a fee of £15.
  • What happens if I miss an appointment?
    PAY AS YOU GO or Chargeable Appointments You can cancel or reschedule appointments up to 24 hours before the appointment. You can do this free of charge through the patient portal or by calling the Practice on 0333 241 6441 during our opening hours. If you cancel the appointment within 24 hours, you will be charged 50% of the full price of the consultation. If you reschedule the appointment within 24 hours, you will incur an administration fee of £15. If you do not attend the appointment and do not inform us, you will be charged the full amount for the missed appointment. Membership Appointments You can cancel or reschedule appointments up to 24 hours before the appointment. You can do this free of charge through the patient portal or by calling the Practice on 0333 241 6441 during our opening hours. If you cancel the appointment within 24 hours or do not attend, you will lose one of your included annual consultations. If you reschedule the appointment within 24 hours, you will incur an administration fee of £15.
  • How can I be seen by a GP?
    You choose how and when you want to be seen – home visits, local surgery appointments, phone or video consultations - depending on your annual subscription plan and what is included.

    12 Lead ECG - £85 – this is a 15-minute appointment that is performed by a member of the nursing team in clinic. Results are available immediately if needed. 24 hour monitoring ECG - £185 48 hour monitoring ECG – £225 72 hour monitoring ECG - £275 The monitoring device will be fitted by a member of the nursing team in clinic. The patient wears the device and carries on with normal activities and returns to the clinic thereafter, where the results are downloaded and reviewed/reported by a Cardiologist within 2 days. For all self-booked ECGs, you will receive a copy of the ECG report along with any recommendations made by the Cardiologist.   • Joint Injections - from £200
    Our GPs offer injections for inflamed, swollen joints in a quick procedure to help ease chronic joint pain and stiffness. We offer pain relief steroid injections for the shoulders, elbows and knees in cases of osteo-arthritis and Golfer's and Tennis elbow. You will need to book a short appointment with a GP to review suitability, this appointment costs £50.   • Cosmetic Treatments (Non-Surgical)
    We are able to offer the following aesthetic treatments (all of our treatments are carried out by registered GPs in our Care Quality Commission (CQC) registered practice): Micro-Needling – great for skin revitalisation, texture improvement, fine line softening, skin elasticity improvement.     There are over 100 varieties of Human Papillomavirus (HPV) and 4 out of 5 people will come into contact with at least one type of the HPV virus at some point in their lives. Most forms of the HPV infection are harmless, however some can cause genital warts and abnormal cells that are linked to cervical cancer and other cancers in men and women. What is the HPV vaccine? The HPV vaccination (Gardasil 9) protects against 9 types of HPV, including 2 types that can cause more than 70% of cervical cancers in the UK and 2 types of the virus that cause 90% of genital warts. Gardasil 9 is given as an injection in the upper arm. The number of doses you need depends on your age.     The aorta is the main blood vessel that supplies blood to your body. It runs from your heart down through your chest and abdomen. In some people - as they get older - the wall of the aorta in the abdomen can become weak. It can then start to expand and form an abdominal aortic aneurysm. The condition is most common in men aged 65 and over and screening on the NHS is not offered to anyone not in this category (i.e. men under 65 or women of any age). If you have an aneurysm, you will not usually notice any symptoms. This means you cannot tell if you have one, will not feel any pain and will probably not notice anything different. We offer screening so we can find aneurysms early and monitor or treat them. This greatly reduces the chances of the aneurysm causing serious problems.   • Fair Usage Policy
    In order to enable Fylde Private GP to offer an efficient and rapidly accessible service to all Members, we need to ensure that excessively high usage by a minority of members is avoided. Therefore, in a few exceptional circumstances, where usage by a member becomes excessively high, Fylde Private GP reserves the right to discuss the level of usage with the member. This may lead to additional fees being charged for subsequent consultations during the membership year and a higher membership fee being charged for subsequent membership years. The new fee will be advised to you prior to renewal.
  • Enforcement of Terms and Conditions
    Failure to enforce or non-reliance on any of these Terms and Conditions by Fylde Private GP will not prevent Fylde Private GP from subsequently relying on or enforcing them. None of the Terms and Conditions or benefits of Membership is enforceable by anyone else other than you.
  • Annual Membership Plan Terms and Conditions
    i. Introduction Welcome to Fylde Private GP and thank you for purchasing your annual Membership Plan. These Terms and Conditions are valid for Members joining on or after 1st October 2021. Please read this information carefully and keep it in a safe place as use of your Membership Plan is subject to these Terms and Conditions. Please note that whilst most of the Terms and Conditions apply to all Membership Plans, there are some variations depending on the type of Plan you have with us. To work out which of the Terms and Conditions apply to your Membership Plan, please make sure you are aware of the type of Plan you hold. ii. Annual Membership Plans This section details the services that are available under the different Membership Plan options. Annual Membership Plans: the contract is for 12 months, and the cover is paid for on a monthly basis, by Debit or Credit Card via Opayo (SagePay). The first month’s payment includes a pro-rata payment for the current month and the whole of the second month. Following the first payment, membership payments will be taken on the first of every month. The initial contract is for 12 months but thereafter runs on a continuous basis until cancelled. The minimum term prior to cancellation for annual membership plans is 6 months. If you do not wish to renew your Annual Membership, or if you wish to switch to a different Membership Plan, you should notify Fylde Private GP prior to your renewal date.   • General Rights to Refuse Service
    Fylde Private GP reserves the right to refuse to provide advice or consultations where: Membership fees have not been paid Outstanding charges have not been paid In its reasonable opinion, the giving of service would involve any breach of the law It reasonably considers that you or anyone accompanying you, is behaving or has behaved in an abusive or threatening manner to Fylde Private GP’s employees or agents It reasonably considers that that you, or anyone acting on your behalf, is behaving, or has behaved, in an abusive or threatening manner to Fylde Private GP’s employees, either over the phone or in writing Please note: if you are refused service by Fylde Private GP you have the right to an explanation in writing (see ‘Compliments and complaints’ for contact details).
  • Changes to Terms and Conditions
    Fylde Private GP is entitled to change any of the Terms and Conditions on the anniversary of your joining date. Fylde Private GP also reserves the right to make changes to these Terms and Conditions during the Membership Year, on the giving of reasonable notice, where it reasonably considers this necessary in order to comply with any applicable laws, regulations or the instruction of a regulatory authority.
  • Appointment Cancellation Policy (Annual Membership Plans)
    You can cancel or reschedule appointments up to 24 hours before the appointment. You can do this free of charge through the patient portal or by calling the practice on 0333 241 6441 during our opening hours. If you cancel the appointment within 24 hours or do not attend, you will lose one of your included annual consultations. If you reschedule the appointment within 24 hours, you will incur an administration fee of £15.

  • Exclusion of Liability For Loss of Profit Etc
    Fylde Private GP shall not, in any event, and to the extent permitted by law, have any responsibility for any increased costs or expenses, for any loss of profit, business, contracts, revenue or anticipated savings or for any special, indirect or consequential losses incurred as a result of or in connection with any service, whether resulting from tort (including negligence or breach of statutory duty), breach of agreement or otherwise. For the avoidance of doubt, nothing in this clause or these Terms and Conditions shall exclude or restrict Fylde Private GP’s liability for negligence resulting in death or personal injury.
  • Changes to Your Personal Details
    Changes to your name, address, telephone number or bank account details must be notified to Fylde Private GP immediately. You can log into the Patient Portal and make these changes yourself or alternatively you can call us during practice opening hours and we can make these changes on your behalf.
  • Cancellation of Annual Membership Plans
    You have the right to cancel your Membership Plan within a 14 day ‘cooling off period’, commencing from the date of your membership and initial payment. Cancellations must be confirmed in writing.
  • Pay As You Go (PAYG) Terms and Conditions - in Addition to the General Terms and Conditions (points 1 - 15), the Following Apply:
    i. These Terms and Conditions are valid for patients using our service on a Pay-As-You-Go basis on or after 01/10/2021. Please read this information carefully and keep it in a safe place as use of our services are subject to these Terms and Conditions. ii. Pricing Please see our website for our Pay As You Go Pricing. If you require additional information or have any questions, please contact the Practice and we will be happy to help you. iii. Appointment Cancellation Policy You can cancel or reschedule appointments up to 24 hours before the appointment. You can do this free of charge through the patient portal or by calling the Practice on 0333 241 6441 during our opening hours. If you cancel the appointment within 24 hours you will be charged 50% of the full price of the consultation. If you reschedule the appointment within 24 hours you will incur an administration fee of £15. iv. Missed Appointments If you do not cancel, or do not attend the appointment without informing us you will be charged the full price of the consultation.
  • National Health Service (NHS) Care
    Members are encouraged to continue their registration with an NHS GP if they are entitled to NHS care. The clinic will liaise with any other healthcare professionals involved in your care, with your consent, as needed.
  • Video Consultations
    If you choose a video consultation, please note that you will be required to download the Meddbase Telemedicine Application to your phone, tablet or PC prior to your first consultation. Once you have made a video consultation appointment, you will be sent an email and text confirming the appointment along with a link to join the consultation. Please ensure that you have downloaded the app before the start of your scheduled appointment time so that this does not delay the start and leave you with less time for your consultation. It is the patient’s responsibility to ensure that this application is downloaded in advance and Fylde Private GP cannot be held responsible for missed or delayed consultations. The day before your telemedicine consultation, our staff will call each patient to remind them of this requirement.
  • Additional Charges - Clinic GP Consultation - 15 Minutes Appointments
    Please note that 15-minute appointments are for minor alignments only. We highly recommend contacting the practice before booking to assess your requirements to confirm the most appropriate appointment type and length. Please note that if your appointment is for a complex issue and overruns the allocated 15 minutes, you will be charged an additional £45 for a 30-minute appointment.
  • The Following Refund Policy Will Apply for Cancelling Within the Cooling-Off Period:
    ii. If you have already had a consultation, you will receive a full refund of the Membership fee paid. iii. If you during the 14 day cooling-off period, you have already used any of our services, you will receive a full refund of the total Membership fee paid less the cost of any charges for our services based on our Pay-As-You-Go Pricing. iv. If you have during the 14 day cooling-off period you have used our services and the cost of these services used are in excess of the Membership Fee paid you will be responsible for paying the cost of the difference between the two. Please note that there will be no additional cooling-off period(s) during the Membership Year, regardless of any changes that are made to the Membership Plan.
  • Home Visits
    Our home visit catchment area is based on a 15-minute drive time radius from our practice. Only by pre-agreement are home visits available to those who live within this catchment area. Based on availability, home visits may be agreed with the practice outside of this radius. However, extra travel time will be charged at a rate of £65 for Doctors and £24 for Nurses per 30 mins, with a minimum charge of 15 mins.   • Interpretation: Use of English Law
    Your Membership and these Terms and Conditions are governed and should be interpreted by the laws of England and Wales.
  • New Patient Health Questionnaire
    Before your first appointment, we will ask you to fill out a health questionnaire. For your safety, this must be filled in as accurately as possible, including details of all your existing medication. To complete this health questionnaire, please log into your member's portal here, go to Questionnaires and complete the questionnaire. Once completed, this will be submitted through the system and will be saved in your patient notes ready for the Doctor to review with you at your first consultation. If you do not have access to the internet or need assistance completing the health questionnaire, please let a member of our staff know and they will be able to assist you when you arrive at the practice. Please ensure that you arrive earlier than your scheduled appointment time to ensure that we have time to help you complete this ahead of your appointment. When a child is registered, the details of those with parental responsibility will also be sought and the parent will need to complete the health questionnaire on behalf of the child.

  • Matters Outside Fylde Private GP's Reasonable Control
    While Fylde Private GP seeks to meet the service needs of members at all times, its resources are finite and this may not always be possible. Fylde Private GP shall not be liable for service failures where Fylde Private GP is faced with circumstances outside its reasonable control.
  • Additional Charges
    Any additional charges for services not included in the consultation will be advised to patients before they are requested by the clinician. An invoice detailing these charges will be raised during the consultation and will be ready for payment before you leave the practice. The prices for individual items are also available through our website or by contacting our reception. If you have any queries or questions regarding fees, please do not hesitate to ask for clarification. Please note additional fees may be incurred for vaccinations, laboratory fees, procedure fees etc. All such fees are payable immediately after the provision of service.

  • If You Wish to Cancel After the Cooling-Off Period Has Expired, the Following Will Apply:
    v. The minimum term prior to cancellation for annual membership plans is 6 months. There will be no refund of the Membership fee for the first six months. The only exception will be in the event of death, in which case we may refund your estate with an amount proportionate to any full months left. vi. You can cancel your cover at any time after the initial 6 months by giving a minimum of 30 days notice. Payment cancellation will take effect at the next payment date following the end of the notice period. You will be advised of the date when your membership expires. vii. Fylde Private GP has the right to cancel any membership plan immediately if you or anyone accompanying you, is behaving or has behaved in an abusive or threatening manner to Fylde Private GP employees or agents, or if, in Fylde Private GP’s opinion the therapeutic relationship has irretrievably broken down or it is clear that the service expectations and demands of the patient are unreasonable in relation to the services provided by Fylde Private GP. In addition, a Membership Plan can be cancelled by Fylde Private GP: viii. Immediately if any month’s Continuous Membership fee or other related charge is overdue; ix. At any time by giving at least 30 days notice: cancellation will take effect at the next payment date following the end of this notice period and no refund will be due.

  • CT Cardiac Calcium Score Scan - £550
    A CT Cardiac Calcium Score scan is a simple Computerised Tomography (CT) scan to check for deposits or build-up of plaque on the walls of your coronary arteries (arteries that supply blood to your heart). It doesn’t provide detailed pictures of the heart or heart arteries but instead measures the amount of calcified, or hardened, plaques in the heart arteries. Plaque is made up of calcium, fat and cholesterol. This is usually explained as a calcium score which provides an assessment of the number of fatty plaques in the heart arteries. A higher coronary calcium score suggests you have a higher chance of significant narrowing in the coronary arteries and a higher risk of a future heart attack. This test gives your GP early warning of the narrowing of the arteries and the risk of a heart attack.   • Elbow and Forearm Scan - £200
    This Elbow and Forearm Scan is to: ● Examine the elbow for a range of conditions. ● Assess the joint, the flexor and extensor tendons, muscles, ligaments, bursa and soft tissue. This scan, available from 16 years of age, is performed by a local sonographer with diagnostic scanning experience. It can help to identify the cause of: ● Elbow pain ● Possible injury ● Inflammation of the tendons For conditions like: ● Tennis and golfer’s elbow ● Olecranon bursitis ● Tendinosis ● Degenerative changes, such as arthritis Please note, the sonographer may want to assess the opposite elbow for comparison. This will be at the discretion of the sonographer and is not always necessary.   • Do I need to join your practice to be seen by a private GP?
    No, you can use our services without being a member. Please note that these services come at an increased price and we prioritise our appointments for membership plan patients.
  • Can you give me an NHS prescription?
    No, we can only issue you with a private prescription that your pharmacy will process in the same way. Some medications are cheaper on a private prescription than with the NHS and others are more expensive. This can vary from pharmacy to pharmacy. You can still get your NHS prescriptions that your NHS GP has prescribed you.
  • Do I need to pay to join?
    There is no joining fee. Our annual membership plans include a range of services that are included in your monthly subscription fee and are therefore free to use. If you choose our pay as you go option, you pay for the service at the time of booking.
  • Will my NHS GP know that I am registered with you?
    No – not unless you consent for us to keep them updated. Any routine investigations and test results are private to you and will not be shared with your GP. Only with your permission will we share your treatment with us with your registered GP. For your own health and safety, we will ask for your consent to share important medical information with your GP.
  • Can I still be registered with my NHS GP and use a private GP?
    Yes, you can. You do not forfeit any of your NHS entitlements by using our service. We recommend that you stay registered with your NHS GP practice to ensure assistance in the event of a medical emergency.
  • Do I need to book an appointment or can I just walk in?
    Appointments are strictly by prior appointment. If you are a member, you can book appointments online through our patient portal on our website.
  • Can I see a GP and not be on an annual membership plan?
    Yes, you can pay as you go for the service that you use. Please note that these services come at an increased price and we prioritise our appointments for membership plan patients. You can join as a member on an annual plan at any time, which may be more cost effective in the longer term depending on your healthcare needs.
  • What about my medical history?
    At the present time, we will not have access to your NHS medical records, but part of your registration process is for the GP to capture this information from you through a new patient questionnaire form. If you decide to join on a Membership Plan, We suggest that you obtain a ‘Brief Summary’ of your medical records from your NHS practice which will load onto your patient record. This is a free of charge service that the NHS can provide you.
